Hi, yes it is custom made, you dont have forms with select nor checkbox options, or at least I could not see them.

I fixed it, however please test the contact form of the template, I believe there is a bug in the js file regarding the subject line(or improve the documentation of the template) and why not instead of using the placeholder when reading the array of fields why not use the title of the fields, I ask this is because not all tags have placeholder attribute and if in my form I have a select or radio button it does not work they way your would expect it to do so.
Also please add in the send.php the little piece of extra code for sending a confirmation email to the user.
This suggestion will make this wonderful template super awesome.
